If you are having any trouble logging in to AYSO or have never logged in before and it has been more than 30 days since your date of hire, please fill out the AYSO Password Reset Form and click submit. Once your account has been re-opened, Payroll Services will email you instructions on how to set it up and gain access. All requests submitted before 3 p.m. will be completed on the same day. Any requests submitted after 3 p.m. will be completed the next business day.
- Separated employees who require access to download/print out W-2 forms, employment verification, 1095Cs or any other items may also use this form. Please ensure your email address is correct before submitting.
- If you are a retiree in need of a copy of your 1099-R (tax form that reports retirement income) but cannot access the website, you may also use this form, or you can contact the Retirement Administration Service Center at (510) 888-8267 to have a paper copy mailed to you.
