Purpose

The GLACIER Tax Summary Report is a summary of the information entered into an individual’s GLACIER record. Payroll Services uses the report to ensure that the appropriate tax status is applied to payments issued by UCLA to the individual, for tax withholding and reporting purposes. The report includes the following information:

  • Residency for tax purposes.
  • Tax treaty eligibility.
  • FICA eligibility.
  • Required forms and documents that need to be submitted to Payroll Services (varies by tax status and type of payment received).

Generating a Report

Individuals with a record on GLACIER (UCLA’s online nonresident alien tax compliance system), can print this form from within their GLACIER record. For information on GLACIER, see Related Information.

Submitting a Report

The Tax Summary Report along with the required forms and documents listed on the Report, should be submitted to Payroll Services at the following address:
